Rails -- Polymorphic comments on deletion of Assignment and Examination

On deletion of an Assignment or Examination, a modal requesting reason should pop up, upon one submitting the reason, deletion of an ActiveRecord and Creation of Comment should take place respectively.

Kindly guide me on how to achieve the above through Polymorphic in detail.
